Most people who walk in unhappy with past injectable work have no complaint about the medication. Their complaint is judgment. Somebody treated every muscle at the same dose, or filled a face that needed relaxing, or chased a look that belonged to someone else.
We start somewhere else. Before anything is drawn up, we look at how your face moves, which lines bother you (not which lines we think should bother you), and what the smallest amount of work is that gets you there. We dose conservatively because that is the version of this you can still change your mind about. You can add more in two weeks. You cannot subtract.
Botox and the other neuromodulators are purified proteins that temporarily quiet the signal between a nerve and a specific muscle. When that muscle relaxes, the skin folding over it relaxes too, and the line softens. It does nothing for volume loss, sun damage, or texture, which is why an honest consult sometimes ends with us recommending a skin treatment instead.
The effect wears off as nerve signaling returns, so nothing about your first appointment locks you into anything.
The muscles between your brows pull inward when you concentrate, squint, or read your phone in the sun. Over years, that fold etches in. Softening those muscles is the single most requested treatment we do, partly because those lines read as angry or tired even when you feel neither.
The horizontal lines come from the muscle that lifts your brows. This is the area where over-treatment shows most, because that same muscle holds your brow position. Treated too heavily, brows drop and the face looks flat. We treat the forehead in relation to the frown lines, not in isolation, which is the part that takes experience rather than a chart.
The lines that fan from the outer corners of your eyes when you smile. Many patients want these softened, not erased, since a smile that does not reach the eyes reads as off. We talk about where on that spectrum you want to land before we treat.
Filler solves a different problem. Where neuromodulators relax movement, fillers restore volume in places that have hollowed or descended: under-eye shadowing, cheeks that have flattened, folds that stay carved in when your face is still, lips that have thinned.
The quick test we use in the room: if a line is only there when you move, that is usually a Botox conversation. If it is there when your face is at rest, that is usually a filler conversation. Plenty of faces need a little of each, and plenty need neither yet.
Overfilled faces happen gradually. Each appointment seems reasonable on its own, and nobody stops to look at the whole face. So we treat filler as a plan: we place a conservative amount, bring you back to look at it in natural light and normal expression, and decide together whether anything else is warranted. Sometimes the answer is that you are done, and we will say that out loud.
Two injectors working from the same syringe produce two different faces. The skill is knowing facial anatomy, where the vessels run, how much a given area can hold, and when to stop.
It matters for safety too. The FDA has been direct that dermal fillers carry real risks, including infection, lumps and nodules, and rarely, filler entering a blood vessel, which can cause tissue damage or vision changes. Florida restricts who may inject and requires an in-person, physician-supervised evaluation for that reason. Telehealth questionnaires do not meet that standard. Filler shows up immediately, but what you see on day one includes swelling, so the honest verdict comes a week or two later.

Both are temporary. Both need maintenance if you want to keep the result. And both vary from person to person: your muscle strength, your metabolism, and your anatomy all change the answer. We will not promise you a number of months or a specific outcome, because nobody can honestly do that.
Contraindications are reviewed individually at your in-person evaluation. If injectables are not appropriate for you, or not appropriate yet, we will say so and point you toward what is.
For most people, typically three to four months. That range moves depending on the area treated, how strong the muscle is, your metabolism, and how much product was appropriate for you. Some patients stretch further between appointments, others come back sooner. Results vary from person to person, and your provider will give you a realistic timeline for your face at your visit.
Not if we do our job. Frozen is usually the result of treating every muscle at a heavy dose instead of the specific muscles causing the lines that bother you. We treat conservatively, keep your expression intact, and ask you to come back in about two weeks so we can add a little if you want more softening. Adding takes ten minutes. Waiting out too much takes months.
